A case of emphysematous pyelonephritis

In this case a 34-year-old woman with poorly controlled diabetes presented with symptoms indicative of a severe pyelonephritis. Following a failed response to initial treatment measures, a diagnosis of EPN was made following a CT scan. With aggressive fluid resuscitation, antibiotics and decompression with nephrostomy, the patient improved and avoided surgical intervention. The diagnosis, classification and treatment of EPN are discussed.
